Safe Discharge Planning

Discharge from a facility is a crucial part of a continuation of care and, with effective planning, the process can lead to a reduction in hospital readmissions.

This process is needed for a smooth transition into homecare that becomes beneficial to both the facility and the client. Therefore, First Choice Homecare works to ensure the transition of care is as comprehensive and thorough as possible.

Our approach includes:

• Conducting an evaluation of discharge planning in a timely manner
• Obtaining and maintaining contact with all providers and care teams
• Ensuring the client is fully verse in all aspects of the discharge orders
• Coordinate transportation to home and/or rehabilitation facilities
• Coordinate appropriate referrals to support groups
• Facilitate Durable Medical Equipment (DME) orders and supplies
• Collection and reconciliation of medications
• Complete a full environmental assessment to ensure home is appropriate for safe discharge to home
